    Analysis of the short-term efficacy and safety of overlap and π anastomosis in robot assisted total gastrectomy

    Objective To explore the short-term efficacy and safety of overlap anastomosis and π anastomosis in robot assisted total gastrectomy. Method 103 patients who underwent total gastrectomy in Hainan Provincial People's Hospital from January 2022 to January 2025 were selected as the research objects and randomly divided into two groups according to the number table method. The Overlap group (n=52) received Overlap anastomosis, and the π group (n=51) received π anastomosis. The operation conditions (operation time, digestive tract reconstruction time, intraoperative blood loss, number of lymph node dissections, anastomosis time), postoperative recovery (postoperative analgesia time, ambulation time, anal exhaust time, postoperative hospital stay), nutritional indicators and adverse reactions between the two groups were compared. Result The operation time and anastomosis time of π group were shorter than those of overlap group (P < 0.05); There was no significant difference in intraoperative blood loss and the number of lymph nodes between the two groups (P > 0.05); The postoperative anal exhaust time and postoperative hospital stay in π group were shorter than those in overlap group (P < 0.05); There was no significant difference in postoperative analgesia time and ambulation time between the two groups (P > 0.05). At 3 months after operation, the levels of hemoglobin, albumin and prealbumin in the two groups were higher than those before operation (P < 0.05), but there was no significant difference between the overlap group and the π group (P > 0.05). The total complication rate in π group was significantly lower than that in overlap group (P < 0.05). Conclusion In robot assisted total gastrectomy, the operation time of π anastomosis are short, the postoperative recovery of patients is fast, and the incidence of complications is low, the short-term curative effect is significant and has high safety.

    Objective To explore the relationship between coping style and health behavior of patients after cholecystectomy and its impact analysis. Method 94 patients who underwent cholecystectomy in Pizhou Dongda Hospital from January 2023 to October 2024 were selected by convenient sampling method. The general information questionnaire, the medical coping style questionnaire and the health promoting lifestyle scale were used to investigate them, and the scores of coping style and health promoting lifestyle of patients after cholecystectomy and their correlation and influence were analyzed. Result the health promoting lifestyle score of patients with cholecystectomy was (106.44 ± 15.94), and the yield dimension score, avoidance dimension score and face dimension score were (7.76±2.15), (9.96±2.37) and (22.85±4.61), respectively. There were signifi cant diff erences in the scores of health promoting lifestyle in patients' economic income (F=2.366, P=0.022), educational level (F=2.588, P=0.015), coping style (F=11.352, P<0.001), smoking (t=4.050, P<0.001), and basic diseases (t=2.305, P=0.026). Economic income, education level, coping style, smoking and basic diseases were the influencing factors of health promoting lifestyle scores of patients after cholecystectomy (P < 0.05). Conclusion There is a significant correlation between health promoting lifestyle and coping style of patients after cholecystectomy.

    Objective To explore the effect of lenvatinib, programmed death-1 (PD-1) inhibitor combined with transcatheter arterial chemoembolization in the treatment of patients with advanced liver cancer. Method 40 patients with advanced liver cancer in Gaozhou People's Hospital from July 2022 to May 2024 were selected and divided into two groups by double-blind method, 20 cases in each group. The control group was treated with transcatheter arterial chemoembolization, while the observation group was treated with combination therapy of lenvatinib and PD-1 inhibitor on the basis of the control group. The clinical efficacy, liver related indicators and adverse reactions of the two groups were analyzed and compared. Result After treatment, the effective rate of the observation group was 75%, higher than that of the control group (40%), the difference was significant (P < 0.05). There was no significant difference in alanine aminotransferase, total bilirubin, aspartate aminotransferase and alpha fetoprotein levels between the two groups before and after treatment (P > 0.05). After treatment, the level of alpha fetoprotein in the observation group was significantly lower than that in the control group (P < 0.001). After treatment, the incidence of rash and diarrhea in the observation group was significantly higher than that in the control group (P < 0.05), but there was no significant difference in the incidence of gastrointestinal bleeding, hypertension and proteinuria between the two groups (P > 0.05). Conclusion Transcatheter arterial chemoembolization combined with lenvatinib and PD-1 inhibitor can improve the treatment effect of patients with advanced liver cancer, reduce the level of alpha fetoprotein, and has high safety.

    Objective To explore the diagnostic value of red blood cell distribution width (RDW) and serum neuron specific enolase (NSE) in colon malignant tumors. Method 154 cases of patients with a clear clinical diagnosis of colon cancer (colon cancer group) and 150 cases of healthy people (control group) were selected from The Fifth Affiliated Hospital of Xinjiang Medical University from 2018 January to 2021 January, and the blood routine indexes of the two groups were compared, including red blood cell (RBC), platelet (PLT), lymphocyte ratio (LY%), RDW and tumor We compared the levels of carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA-125), carbohydrate antigen 125 (CA-199), and NSE in colon cancer patients with diff erent clinical data. RDW and NSE levels in colon cancer patients with diff erent clinical data. The diagnostic value of RDW, NSE, CEA and CA19-9 for colon cancer was analyzed by applying receiver operating characteristic (ROC) curve. Result RBC, LY%, RDW in blood routine, NSE, CEA, CA125, CA19-9 in tumor markers were statistically significant compared with the control group, and there was a significant correlation between the positive expression of serum RDW and the pathological stage of the tumor and whether lymph node metastasis had occurred, and it was concluded that RDW, NSE and its combination with CEA and CA19-9 had high diagnostic value in colon cancer by ROC curve. The ROC curve showed that RDW, NSE and their combination with CEA and CA19-9 had high diagnostic value for colon cancer. Conclusion Elevated levels of RDW and NSE may have a close relationship with the development of colon cancer, and both of them may become an important indicator for the early diagnosis and judgment of colon cancer.

    Objective To compare postoperative quality of life between Billroth Ⅱ and uncut Roux-en-Y anastomosis in patients undergoing radical distal gastrectomy for gastric cancer. Method The clinical data of 60 patients who received radical distal gastrectomy in Dongfang Hospital Affiliated to Tongji University from November 2019 to August 2022 were collected. The patients were divided into two groups according to the postoperative digestive tract reconstruction: Billroth Ⅱ anastomosis group (B- Ⅱ group) and uncut Roux-en-Y anastomosis group (uncut R-Y group). The Chinese version of the postgastrectomy syndrome assessment scale (PGSAS)-37 was used to evaluate and compare the differences between the two groups in the field of symptoms, life status and quality of life. Result A total of 60 patients were included in this study, including 26 patients in group B- Ⅱ and 34 patients in uncut R-Y group. There were no significant differences in age, gender and postoperative follow-up time distribution between the two groups (P > 0.05). In terms of surgical methods, the proportion of laparoscopic surgery (including laparoscopic assisted surgery and total laparoscopic surgery) in the uncut R-Y group was significantly higher than that in the B- Ⅱ group (χ2=17.532, P=0.001). PGSAS-37 score showed that the gastric acid reflux symptoms in the uncut R-Y group were significantly better than those in the B-Ⅱ group (P=0.044); There was no significant difference in other symptoms, life status and quality of life between the two groups (P > 0.05). Conclusion Uncut Roux-en-Y anastomosis demonstrated superiority over B- Ⅱ anastomosis regarding acid reflux symptoms after distal gastrectomy. However, no significant differences were found in other quality of life aspects.

    Objective To study the effect and safety of laparoscopic middle approach and caudal medial approach in the operation of right colon cancer. Method 85 patients with right colon cancer in Yulin Third People's Hospital from April 2022 to April 2024 were selected. All patients received radical resection of right colon cancer. They were divided into control group and observation group according to different approaches. The control group (n=42) was treated with intermediate approach surgery, and the observation group (n=43) was treated with oneway loop surgery via caudal medial approach. Result The number of lymph node dissection in the observation group was more than that in the control group, and the operation time was shorter than that in the control group, and

    the intraoperative blood loss was less than that in the control group, the differences were significant (P < 0.05). After operation, the carcinoembryonic antigen, carbohydrate antigen 125, carbohydrate antigen 19-9, cortisol, adrenocorticotropic hormone, norepinephrine in the observation group were signifi cantly lower than those in the control group (P < 0.05). The conversion rate of laparotomy (0 vs 9.52%) and the total complication rate (6.98% vs 23.81%) in the observation group were lower than those in the control group, and the differences were significant (P < 0.05). Conclusion The medial caudal approach of one-way loop laparoscopic surgery for right colon cancer needs shorter time, less intraoperative blood loss, which is more conducive to the postoperative recovery of patients, and the operation idea is clear, which can avoid repeated overturning of the mesentery and intestines, and is more conducive to the completion of lymph node dissection.

    Objective To investigate the effect of single incision laparoscopic radical resection of colon cancer on postoperative immune ability and stress response in elderly patients with colorectal cancer. Method A total of 68 patients with colorectal cancer who received single incision laparoscopic treatment in the second geriatric hospital of Wuxi from May 2022 to May 2025 were selected and randomly divided into two groups, 34 cases in each group. The conventional group received conventional open surgery, and the experimental group received single incision laparoscopic radical resection of colon cancer. The physical signs, cellular immune function and stress response index levels of the two groups at different time points were compared. Result There were no signifi cant differences in heart rate, mean arterial pressure, pulse oxygen saturation (SpO2) and respiratory rate between the two groups before anesthesia, when anesthesia was successful and 72 hours after operation (P > 0.05). At the end of operation and 24 hours after operation, the heart rate of the experimental group was lower than that of the conventional group, and the mean arterial pressure, SpO2 and respiratory rate of the experimental group were higher than those of the conventional group, with significant differences (P < 0.05). There were no signifi cant diff erences in the percentages of CD3+T cells, CD4+T cells, cortisol and interleukin-6 between the two groups before and after anesthesia (P > 0.05). At the end of operation, 24 hours and 72 hours after operation, the percentages of CD3+ T cells and CD4+ T cells in the experimental group were higher than those in the conventional group, and the levels of cortisol and interleukin-6 were lower than those in the conventional group, with significant differences (P < 0.05). Conclusion Compared with traditional open surgery, single incision laparoscopic radical resection of colon cancer has less impact on elderly patients with colorectal cancer, can better stabilize the immune ability of patients, and the stress response is mild.

    Objective To analyze the application value of endoscopic retrograde appendicitis surgery in the treatment of acute uncomplicated appendicitis. Method From January 2023 to December 2024, 82 patients with acute uncomplicated appendicitis in Liangxiang Hospital of Fangshan District in Beijing were selected and randomly divided into two groups, 41 cases in each group. The control group was treated with laparoscopic appendectomy, and the observation group was treated with endoscopic retrograde appendectomy. The differences of perioperative related indexes between the two groups were compared, and the changes of infl ammatory reaction and oxidative stress indexes before and after operation were compared between the two groups.     Objective To investigate the value of ultrasound real-time monitoring in early complications after appendectomy. Method The clinical and ultrasonic data of 60 patients with complications after appendectomy in Guilin People's Hospital from January 2023 to December 2024 were retrospectively analyzed. The sonographic characteristics of various complications were recorded and summarized. Result Among the 60 patients with complications after appendectomy, there were 16 cases of ascites, 12 cases of abdominal abscess, 23 cases of incision infection, 6 cases of appendicitis stump, and 3 cases of intestinal obstruction. The diagnosis was verified by the results of operation, puncture or clinical follow-up. The ultrasonic characteristics of ascites were clear-cut anechoic or hypoechoic areas, which were mostly banded or irregular flakes; The ultrasonic characteristics of abdominal abscess were mixed echo masses with irregular boundaries, uneven internal echo, flocculent, reticular or cord like strong echo, some of which were combined with liquid-liquid plane or bubble like strong echo and accompanied by rear "comet tail sign" sound shadow; The ultrasonic features of incision infection were thickened and disordered echo of subcutaneous tissue, blurred hierarchical structure, and patchy or irregular hypoechoic areas were seen locally; The ultrasonic characteristics of appendicitis stump were as follows: a cord like blind end structure appeared in the area adjacent to the root of the cecum, the diameter was thickened, the average transverse diameter was 8.4mm (range 7.2 ~ 10.1mm), low to medium echo, uneven internal echo, and spot or strip hyperechoic in the lumen could be seen; The ultrasonic characteristics of intestinal obstruction were typical intestinal dilatation and fluid gas accumulation.     Value analysis of high-resolution magnetic resonance imaging in preoperative T staging evaluation of locally advanced rectal cancer

    Objective To explore the sensitivity of high-resolution magnetic resonance imaging (MRI) in the preoperative T staging assessment of locally advanced rectal cancer. Method A total of 86 patients with locally advanced rectal cancer who underwent preoperative high-resolution MRI staging at the First Hospital of Hohhot from February 2024 to January 2025 were included and compared with the postoperative pathological results. The sensitivity, specificity rate, accuracy rate, positive predictive rate, negative predictive rate and Kappa value were calculated to evaluate the accuracy and consistency of high-resolution MRI in diagnosing T staging. The receiver operating characteristic (ROC) curve was plotted and the area under the curve (AUC) was calculated. Result Among the 86 patients with locally advanced rectal cancer, 76 cases had the same high-resolution MRI and pathological stage. The overall diagnostic accuracy rate was 88.37%, and the Kappa value was 0.842, showing a high degree of consistency with the postoperative pathology. High-resolution MRI had the highest diagnostic sensitivity (92.86%) in stage T3, but its specificity was relatively low (80.00%). The sensitivity of stage T4 was 82.61%, the specificity was 95.24%, and the accuracy rate was 91.86%. The specificity of stage T1~2 reached 98.73% and the accuracy rate was 96.51%. ROC curve analysis showed that the AUC of MRI in diagnosing T1~2 stage, T3 stage and T4 stage was 0.851, 0.865 and 0.889 respectively, all of which had high discriminative ability. Conclusion High-resolution MRI has good diagnostic efficacy in the preoperative T staging assessment of locally advanced rectal cancer, especially showing high sensitivity and accuracy in the identifi cation of T3 and T4 stages.

    Objective To explore the impact of zhenqi fuzheng granules combined with probiotics on intestinal mucosal barrier function in patients undergoing nutritional support following early gastric cancer surgery. Method From August 2021 to August 2023, 100 patients with early gastric cancer in Huai'an First Hospital Affiliated to Nanjing Medical University were selected and randomly divided into two groups, 50 cases in each group. The control group was treated with early postoperative enteral nutrition combined with probiotics, while the observation group was treated with zhenqi fuzheng granules based on this. The postoperative recovery indexes of the two groups were compared, and the changes of nutritional status and intestinal mucosal barrier function before and after intervention were compared. Result The recovery time of bowel sounds, the first exhaust time and the hospitalization time of the observation group were shorter than those of the control group, the difference was significant (P < 0.01), but there was no significant difference in the indwelling time of catheter and the first ambulation time between the two groups (P > 0.05). Before intervention, there was no significant difference in the levels of nutritional indicators and intestinal mucosal barrier function related indicators between the two groups (P > 0.05). After the intervention, the nutritional index levels of the two groups were improved, and the levels of transferrin, albumin and prealbumin in the observation group were higher than those in the control group; The levels of intestinal mucosal barrier function related indicators in the two groups were improved, and the levels of intestinal fatty acid binding protein, procalcitonin and D-lactic acid in the observation group were lower than those in the control group, with significant differences (P < 0.001).     Objective To analyze the effect of rapid rehabilitation nursing on the psychological state and pain of patients with modified radical mastectomy for breast cancer. Method From November 2022 to December 2024, 80 patients with breast cancer in Jianli People's Hospital were selected and randomly divided into observation group and control group, 40 cases in each group. The control group received routine nursing intervention, while the observation group received rapid rehabilitation nursing. The psychological status, pain degree and upper limb function recovery of the two groups were compared. Result Before intervention, there was no significant difference in self-rating depression scale (SAS), self-rating anxiety scale(SDS) scores, Fugl-Meyer assessment scale upper extremity (FMA-UE) and motricity index upper extremity(MI-UE) scores between the two groups (P > 0.05). After the intervention, the SAS and SDS scores of the two groups were lower than those before the intervention, and the observation group was lower than that of the control group, the difference was significant(P < 0.05), and the FMA-UE and MI-UE scores of the two groups were increased, and the observation group was higher (P<0.05). The visual analogue scale scores of the observation group were lower than those of the control group at 6h, 12h, 1d and 3d after operation, and the difference was significant (P < 0.05). Conclusion Rapid rehabilitation nursing for patients with breast cancer undergoing modified radical mastectomy can significantly reduce their negative emotions and pain, and improve the recovery of upper limb function.

Method 102 patients with hepatobiliary surgery in the Zhongda Hospital Affi liated to Southeast University from January 2023 to December 2024 were selected and randomly divided into observation group and control group, 51 cases in each group. The observation group was given multi-dimensional nursing intervention, and the control group was given conventional nursing intervention. The anxiety level, recovery and postoperative complications of the two groups were compared. Result The first time out of bed, eating, anal exhaust, defecation time and postoperative hospital stay of the observation group were shorter than those of the control group, and the anxiety score of the observation group was lower than that of the control group, with significant differences (P < 0.05). The observation group had 1 case of bleeding, and the control group had 2 cases of bleeding, 2 cases of catheter slippage, 1 case of bile duct rupture, 1 case of biliary fistula, and 1 case of infection. The total incidence of postoperative complications in the observation group (1.96%) was lower than that in the control group (15.69%), and the difference was significant (χ2=4.387, P=0.036). Conclusion Multidimensional nursing intervention can reduce the anxiety level of patients with hepatobiliary surgery, shorten the recovery time and reduce the incidence of postoperative complications.

    Objective To explore the effect of incentive nursing on patients with rectal cancer surgery. Method 74 patients with rectal cancer who were treated in Jingzhou Gong'an County People's Hospital from December 2022 to December 2023 were selected and divided into the control group and the observation group by double color ball method, 37 cases in each group. The former received routine nursing, while the latter received incentive nursing. The self-care ability, psychological status, hope level, quality of life and satisfaction were compared. Result Before nursing, there was no significant difference in self-maintenance ability scale for adults(ESCA) score, self-rating anxiety scale(SAS) score, self-rating depression scale(SDS) score and hope level score between the two groups (P > 0.05). After nursing, the ESCA score and hope level score of the two groups were improved compared with those before nursing (P < 0.001), and the ESCA score and hope level score of the observation group were higher than those of the control group, while the SAS score and SDS score of the observation group were lower than those of the control group, with significant differences (P < 0.001); In terms of quality of life, the scores of the observation group were higher than those of the control group, and the difference was significant (P < 0.001). The satisfaction of the observation group was 97.30%, which was higher than that of the reference group (83.78%), and the difference was significant (P < 0.05). Conclusion The application of incentive nursing in patients who undergone surgery for rectal cancer has good effects. While improving their self-care ability, it can also promote the improvement of their subsequent quality of life.

    Objective To observe and analyze the effect of evidence-based nursing in the treatment of elderly patients with hernia surgery. Method From January 2023 to September 2024, 100 elderly patients who underwent hernia surgery in Bengbu Third People's hospital were randomly divided into control group (50 cases) and observation group (50 cases). The control group received nursing according to the traditional nursing process, while the observation group received preoperative, intraoperative and postoperative nursing combined with the concept of evidence-based medicine on the basis of conventional nursing. The postoperative recovery, complications and quality of life of the two groups were compared. Result The postoperative exhaust time, ambulation time and hospitalization time of the observation group were shorter than those of the control group, and the differences were significant (P < 0.05). Before intervention, there was no significant difference in the quality of life score between the two groups. After intervention, the quality of life score of the observation group was significantly higher than that of the control group, and the incidence of complications in the observation group was 6.00%, lower than that of the control group (16.00%), with significant differences (P < 0.05). Conclusion The application of evidence-based nursing in elderly patients with hernia during operation can significantly improve the speed of postoperative recovery, improve postoperative quality of life, reduce the incidence of complications.

    Objective To explore the application effects of a Chinese-English bilingual teaching model in the clinical internship course of colorectal cancer. Method A total of 50 medical students who participated in clinical internships at the Gastrointestinal Surgery Department of Jiangmen Central Hospital from January 2024 to January 2025 were selected as research subjects. The bilingual group (n=25) received bilingual instruction, while the control group (n=25) was taught using traditional Chinese methods. The mastery of theoretical knowledge, literature reading and analysis skills, and clinical practice abilities were compared between the two groups. Additionally, a questionnaire survey was conducted to assess the satisfaction levels regarding their respective teaching models. Result The bilingual group demonstrated significantly higher mastery of literature reading and analysis skills, as well as clinical practice abilities compared to the control group, the bilingual group reported higher satisfaction with the teaching model, showing a statistically significant difference (P < 0.05). Conclusion The implementation of a Chinese-English bilingual teaching model in the clinical internship course of colorectal cancer is beneficial for medical students to grasp the clinical theories and the current status of international cutting-edge research on colorectal cancer, while also further stimulating their interest in learning.
