咀嚼口香糖对腹腔镜左半结肠癌切除术后胃肠功能的影响及机制研究

摘要:

目的 观察咀嚼口香糖对腹腔镜左半结肠癌切除术后患者胃肠功能的影响,并探讨其作用机制。方法 收集2018 年1 月至2020 年9 月农垦中心医院普外科65 例接受腹腔镜左半结肠癌切除术患者,随机分为研究组(n=35)和对照组(n=30),研究组术后完全清醒6h 开始咀嚼2片木糖醇口香糖,每次30min,每天3 次。对照组术后常规治疗。两组术后第1、3 天抽取外周血检查降钙素原(procalcitonin,PCT)和C 反应蛋白(C-reactive protein,CRP)水平,比较两组术后排气、排便时间、术后住院天数、术后炎症因子水平。结果 65 例患者均顺利康复出院,研究组术后排气时间、排便时间、术后住院天数均短于对照组(P<0.05), 术后第1、3 天PCT、CRP 均低于对照组(P<0.05)。结论 腹腔镜左半结肠癌切除术后患者咀嚼口香糖可促进胃肠功能早期恢复,其作用机制可能是抑制机体炎症因子的释放,促进胃肠动力。

Abstract:

Objective To investigate the gastrointestinal function of chewing gum after laparoscopic resection of left colon cancer and explore its mechanism of action. Methods 65 patients underwent laparoscopic resection of left colon cancer were selected in the department of general surgery between January 2018 to September 2020 in Nongken Central Hospital. These patients were divided into research group (n=35) and control group (n=30). The patients in the research group chewed 2 pieces of xylitol gum three times daily for 30 minutes after surgery, while control group received traditional managements. The levels of procalcitonin (PCT) and C reactive protein (CRP) were examined with peripheral blood on day 1 and 3 after operation. Postoperative exhaust and defecation time, postoperative

hospital stays and postoperative inflammatory factor level of the two groups was compared. Results All the 65 patients recovered and were discharged smoothly, and the postoperative exhaust and defecation time and postoperative hospitalization days in the research group were shorter than those in the control group (P<0.05). The levels of inflammatory factors in postoperative day 1 and day 3, PCT and CRP were lower than the control group(P<0.05). Conclusions Chewing gum after laparoscopic resection of the left colon cancer can promote the early recovery of gastrointestinal function, and its mechanism may inhibit the release of inflammatory factors and promote gastrointestinal motility.
